A TYPICAL PRESENTATION OF CONTACT LENS INDUCED PERIPHERAL ULCERS-MULTIPLE FOCAL CORNEAL INFILTRATES

Purpose:

Contact Lens Induced Peripheral Ulcer (CLPU) typically presents as a single
well-circumscribed, circular, dense focal corneal infiltrate involving the anterior corneal layers.Here we report the incidence, and the clinical features of CLPU where more than one focal infiltrate was seen during the event.

Method:

721 subjects with no prior contact lens wearexperience were seen in prospective clinical trials involving lens wear on 6N and 30N EW with low Dk disposable hydrogel lenses and High Dk silicone hydrogels.

Results:

A total of 119 events of CLPUs (definite, probable and possible) were seen in these studies. Three definite events (2.5%) presented with multiple focal infiltrates at the time of the event. While two events presented with two focal infiltrates each, the remaining event presented with 3 infiltrates In addition another event. An additional event (0.8%) presented with 3 dense focal opacities suggesting a resolved event. All patients were symptomatic and reported redness, irritation and watering. All infiltrates were subepithelial and had full thickness loss overlying the infiltrate. A corneal biopsy was taken of one of the infiltrates and histopathology revealed epithelial loss, an intact Bowman’s membrane and infiltration of the anterior stroma with neutrophils. No microorganisms were seen. The other 2 active events were simply discontinued from lens wear and monitored and resolved by Day 12.

Conclusions:

While CLPUs typically present with a single focal infiltrate, a small percentage of events can present with multiple focal infiltrates. Except for the presence of multiple focal infiltrates, the clinical features and the course of the condition is similar to events with single CLPU.
